The role of the rehabilitation team is to support the patient and carers as part of the Hospice multi-disciplinary team. The primary aim is to help the patient achieve their maximum potential function within their own limits and within a realistic time scale, adjusting goals and treatments according to their abilities and stage of illness.
The rehabilitation team aim to:
- give positive psychological support
- set realistic goals and incentives to achieve agreed goals
- motivate patients to maintain reasonable functional activity
- work with carers to enable them to cope more effectively
