
The Hospice holds two outpatient clinics per week, at which patients are seen by the Consultant in Palliative Medicine or another member of the medical team, often jointly with a member of the Day Hospice nursing team.
For some patients, the outpatient clinic may be their first introduction to Hospice services.However, others may be offered a clinic appointment if their Community Palliative Care Nurse Specialist feels they would benefit from medical review, or as follow-up after discharge from the In-patient Unit or Day Hospice.The doctor will communicate with other members of the Hospice team, the patient’s GP, District Nurse, Hospital Consultant and other professionals as appropriate.
