specialist community nursing team

The Specialist Community Nursing Team provides support for patients and their carers either in their own home, nursing home or residential home. They work closely with the patient’s Consultant, G.P. and the District Nurses providing advice on issues relating to the patient’s illness.

They provide specialist assessment of symptoms and emotional and psychological support for patients and their carers.

When appropriate they will refer patients to other Hospice services.
