Speak to the healthcare professional most involved in your care, such as:
You can ask them to make a referral to us via WHISPAR (Wigan Healthier Partnership Integrated Specialist Palliative Care Active Response).
Please note: Patients cannot refer themselves for clinical services. However, those that are bereaved (when the patient has been known to hospice services) can self refer by contacting the clinical secretaries.
When you are referred to the hospice through WHISPAR (Wigan Healthier Partnership Integrated Specialist Palliative Care Active Response), your referral will be assessed by the team who will decide which community services best meet your individual needs. This may mean you receive care in your own home or usual place of residence.
Every referral is carefully considered to ensure you receive the right care, in the right place, at the right time.
