What is Hospice in your Home?

Our Hospice in your Home (HiyH) service has been developed to provide compassionate care for people in the comfort of their own homes.

We aim to offer support that complements the care already being provided by other health and social care professionals.

By adding an extra layer of personalised care, we focus on meeting the unique needs of our patients and their families or carers.

Our dedicated team includes qualified nurses, assistant practitioners, healthcare assistants, and volunteers – all working together to support patients and those important to them, right where they feel most at ease.

How does the service work?

This Hospice in your Home (HiyH) service is designed to support people who are housebound and need care in their own homes.

The team operates during the day from 8:30am to 8pm, and overnight from 10pm to 8am.

Qualified staff carry out an initial assessment and review patient needs every 12 weeks to ensure care remains appropriate and responsive.

When visiting, the team can provide:

  • Personal care to supplement existing care packages at the end of life or following hospital discharge, until a full care package is in place.
  • Carer breaks, giving informal carers time to rest and recharge.
  • Support with future planning, helping patients and families prepare for what lies ahead.
  • Guidance for families adjusting to life with a life-limiting illness.
  • Emotional support, offering a listening ear and compassionate presence.
  • Limited overnight support, allowing carers to get some sleep while knowing their loved one is being cared for.

Once you’re referred to our service, one of our team members will visit you at home to talk about how we can support you and agree on when and how often we’ll visit.

Please note: Your GP and the District Nursing Service will remain responsible for your overall care.
