Hospice Nurse Specialists

Hospice Nurse Specialists

This team has specialist knowledge and skills in palliative care and supports patients with advanced illness, as well as their families and carers.

The Hospice Nurse Specialists provide advice and support by telephone, in clinics at the Hospice's Woodview Centre and offer home visits when patients are unable to attend clinics.

Who are the Hospice Nurse Specialists team?

Our Hospice Nurse Specialists are trained in specialist pallitaive care. They are Wigan and Leigh Hospice staff and are not part of Macmillan Cancer Support.

What do the Hospice Nurse Specialists do?

• Assess symptoms, such as pain, sickness and breathlessness and advise patients, GPs and District Nurses on how these can be managed. Some of our team members are able to prescribe medication that help with symptom management.
• Listen to the worries and concerns of patients and their carers that have arisen due to their advanced illness and provide emotional and psychological support.
• Provide patients and carers with information relating to their illness and about the additional support services available.
• Advise on issues relating to patients' illnesses, such as planning for their future care needs.
• Liaise closely with other health professionals involved in our patients' care and refer them to other services available within the Hospice.

Our Hospice Nurse Specialists do not offer 'hands on' nursing care which is instead provided by district nurses, social services or our Hospice In Your Home service.

Sharing Information
In order to ensure that all of our patients receive the best possible standards of care, the team meets every week with other relevant healthcare professionals at the Hospice to discuss the patients in our care. If a patient does not wish to be discussed in this meeting they can inform their Nurse Specialist.

How are patients referred to our service?
Referrals are accepted from patients' GPs, District Nurses or other senior healthcare professionals.

How can we be contacted?
Team members can be contacted at Wigan and Leigh Hospice on 01942 525566. The Nurse Specialists work between 8.30am to 4.30pm from Monday to Friday but are available to speak to directly between 8.30am and 10am. After 10am, when the team are caring for patients, medical secretaries will take messages which the team will respond to as soon as possible.

At weekends and public holidays, a reduced service operates between 9am and 5pm.

When are patients discharged from this service?
Patients are discharged from the service when:
• They no longer have problems that require support from the Hospice Nurse Specialist Team.
• The patient moves out of Wigan Borough.

If, following a patient's discharge, our services are required again, the patient can either contact us directly (if it is within 3 months of discharge) or discuss a re-referral with their GP, District Nurse or Hospital Team.
