Social Worker

Our specialist palliative care social worker works closely with patients and those people important to them.

Our resident social worker provides psychosocial support and works with the wider hospice team to ensure ongoing care as well as comprehensive, safe and timely discharges from the inpatient unit. Our social worker also arranges formal packages of care.

Patients who are supported in the community, rather than on the inpatient unit, may also benefit from support and advice. The social worker also has links with external agencies which can support our patients helping them to make useful connections.

The hospice recognises it has a duty of care to protect those who are vulnerable to different forms of abuse. Our social worker is trained in safeguarding adults and will work closely with the patient during this time.
